Click on the appropriate one (in this case, Pro Tools). Once you've created your iLok account, you'll need to link it to your Avid account:

When you're done, accept the Terms and Conditions and click the Create Account button.

If you haven't got an iLok account already, you'll need to create one: After a software license is placed on an iLok, you can use it to authorize that software on any computer. Pro Tools and plug-ins require licenses, and Avid uses PACE anti-piracy software to manage them.
